IMPORTANT things about asthma

*Clinical Manifestation of severe asthma :-
1-unable to speak.
2-R.R > 25 /m.
3-P.R > 110 /m.
4-peak flow rate < 50% best known or predicted value.

* Clinical Manifestation of life threatining Asthma :-
1-silent chest.
2-cynosis.
3-bradycardia.
4-hypotention.
5-poor resp. effort.
6-exhausion,confusion or coma.
7-pfr< 33% best known or predicted.
9-sever hypoxia despite oxygen: < 8kpa or 60 mmhg.
10- low ph.

Severity of asthma :- ( new )
(note: it is some what difficult to remember the numbers but i made it so simple although it still difficult)

1- mild intermittent < 2/week <2/month > 80%

2- mild persistent 3-6/w 3-4/m < 80%

3- moderate """" daily > 5/m <80% but
> 60%

4- severe """""" continuous frequent < 60%

Simple plan for E.R management :-
A- Acute asthma attack:-
1- O2 40-60% with salbutamol ( 5mg adult,2.5mg child) via neubilizer mask.
2-If not improving repeat q 15-30 min.
3-"""""""""""""""" add:-
a- ipratropium 0.5mg via neubilizer. if not
b- hydrocortisone 200mg i.v.
4- if not improving consider aminophylline infusion unless pt. is taking oral aminophylline or salbutamole IV 250~g
5- if not it is not a responsibilitu of med.student or H.O ----~~ ICU.
( just kidding)

*-With life threatining asthma do all the above

go to current medical book 2000 table 9-6 page 277 or any newer edition.

Don,t forget the side effect of the used drugs .
? what cause tachycardia in treated patient.
? why some times espically in ped. we give KCL in treated pt. esp. those with some what prolonged treatment. ??
